RING OF STEEL Action Theatre @ Ringstar Studio, LLC.

INFORMED CONSENT AND PARTICIPATION RESPONSIBILITIES I understand my responsibility to:

1. Clear any health issues or medications that are relevant to participation in the Ring of Steel classes,
events and training.
2. Cease practice and report promptly any unusual feelings (e.g. chest discomfort, nausea, difficulty
breathing, apparent injury) during my participation in any activity at the Ring of Steel classes, events
and training at the Ringstar Studio, LLC.
3. Use all practice weapons only for the fencing/stunt work/stage combat/martial practice use for which
they are intended and not to cause harm or damage of any nature.
ASSUMPTION OF RISK I understand that the inherent risks of participating in stunt work, stage combat,
fencing or martial activities, vary with the activity, muscle group involved, and the equipment used. As noted
above, common minor risks include minor muscle strains, muscle strains, muscular fatigue, contusions, and post
exercise soreness. More serious, but less frequent risks include joint injuries, torn muscles, heat related
injuries, and back injuries. There is also the risk of a catastrophic incident (e.g. stroke, heart attack,
paralysis, or death).
I have read the above paragraphs and I know and understand the nature of all of the activities. I understand
the demands of these activities relative to my physical condition and skill level and I acknowledge the types of
injuries that may occur as a result of the activities. I HEREBY ASSERT THAT MY PARTICIPATION IS
VOLUNTARY AND THAT I KNOWINGLY ASSUME ALL SUCH RISKS.
WAIVER OF LIABILITY In the consideration for the use of the services and facilities of the Ringstar
Studio, LLC, the Ring of Steel Action Theatre and its instructors, administrators, and support personnel for
classes and training, I (on behalf of myself, my heirs, personal representatives) do hereby release, waive, and
discharge the Ringstar Studio, LLC, its directors, employees and/or volunteers, the Ring of Steel Action
Theatre and its instructors, from liability for any and all claims that arise from my participating in classes and
training aforementioned (including, but not limited to, organized training activities, classes, individual use of
equipment); and for any and all claims arising from the damage or loss or theft of personal property.
COVENANT NOT TO SUE AND INDEMNIFICATION AGREEMENT I agree not to sue the Ringstar Studio,
LLC, its directors, officers, employees and/or volunteers and the Ring of Steel, or its instructors for any
present or future claims. I also agree to hold harmless, defend and indemnify the Ringstar Studio, LLC and the
Ring of Steel and its instructors from any and all claims arising from my participation in any class, program, or
activity.
ACHKNOWLEDGEMENT OF UNDERSTANDING I have read this Informed Consent, Assumption of Risk,
Waiver of Liability, Covenant Not to Sue and Indemnification Agreement and fully understand its terms. I
understand that I am giving up substantial rights, including my right to sue. I further acknowledge that I am
signing the agreement freely and voluntarily, and intend my signature to be complete and unconditional release
of liability, to the greatest extend allowed by law in the State of Michigan.
