AFFIDAVIT

I, LESTER BARUT, of legal age, Filipino, and with office address at

8th Floor, IMET BPO, Metropolitan Park, Bay Area, Roxas Blvd., Pasay
City, after having been sworn to in accordance with law, hereby depose
and state the following:

1. I am the authorized representative of Topjuan Consulting Inc. to

process all documentary requirements of the Bureau of
Immigration;

2. I am applying for renewal of my accreditation with the Bureau of

Immigration (BI).

3. That I have no relatives within the third (3rd) degree either of

consanguinity or affinity who is an incumbent employee in the
Bureau of Immigration;

4. That should the Bureau of Immigration ascertain that I have

relatives, the Bureau of Immigration has the right to cancel my
accreditation.

5. That this affidavit is executed to attest to the veracity of the

foregoing statements.
