Subject: SHOW CAUSE NOTICE

WHEREAS, I, Zahid Akhtar Zaman, Secretary, Primary &

Secondary Healthcare Department/Competent Authority under the Punjab
Employees Efficiency, Discipline and Accountability Act, 2006 and in due
consideration of facts of the case, am of the view that there are sufficient grounds
to proceed against you on the charge of willful absence from duty w.e.f.
04.08.2015 to till date, which constitute "Inefficiency and Misconduct" within the
meaning of Section-3 of the Act ibid.

AND WHEREAS, the undersigned is of the opinion that it is not

necessary to hold an inquiry into the matter in view of the provisions contained in
Section 5 (1) (b) of the Act ibid. It is, therefore, proposed to proceed against you,
under Section 7 (b) read with Section 5(1) (a) of Punjab Employees Efficiency,
Discipline and Accountability Act, 2006.

NOW, THEREFORE, you are hereby called upon to show cause in

writing within seven (07) days (or within such period as may be extended by the
competent authority) of the receipt of this notice as to why one or more of the
penalties, as prescribed under Section 4 of the Punjab Employees Efficiency,
Discipline and Accountability Act, 2006, should not be imposed upon you.

Your reply to this show cause notice should reach the undersigned
within the stipulated period failing which it shall be presumed that you have no
defence to offer or you have declined to offer the same and have accepted the
charges.
 In case, you desire to consult any record, on which the aforesaid
charges are based and it is relevant to the aforesaid charges, you may do so with
prior arrangement with the undersigned within seven (07) days of the receipt of
this notice.

If the reply to this show cause notice is not received within the
stipulated period, an ex-parte decision shall be taken against you as provided
under the Act ibid.
