INSTRUCTIONS TO THE CANDIDATES

1) The application shall be made in the prescribed format -General purpose form available in the website or can be had from
University forms store. Add Rs 20/- if application form is downloaded from website.

2) The prescribed fee for each copy of transcript is Rs 1000/- (One thousand rupees only).

3) The fee may be paid in cash at University cash counter by means of pay in slip to the credit of Kerala University fund. Remittance
can also be made by demand draft of State Bank of India with an additional amount of Rs 10/- as service charge drawn in favour of
the Finance officer, University of Kerala payable at Trivandrum. Money orders/cheques and postal orders will not be accepted.
Please enclose original cash receipt/DD along with the application form

4) The candidates please note that the transcript shall be taken in a single sheet (both sides) legal size paper. One additional copy of
the transcript form prepared shall be submitted in addition to the required numbers.

5) All the supplementary/failed mark lists shall form part of transcript.

6) Enclosures

a) Application form in the prescribed format.

b) Original fee receipt/ Chalan/ Demand draft.
c) Copy of SSLC/ Higher secondary certificate in proof of date of birth.
d) Copy of degree certificate and all mark lists.
e) Academic Records Request form if required in duplicate and envelope with reference number shall be accompanied if the
transcript has to be given in a sealed cover under the official seal of Registrar.
