A RESOLUTION RESPECTFULLY REQUESTING THE HONORABLE GOVERNOR OF THE PROVINCE OF LEYTE

HON. LEOPOLDO DOMINICO "MIC" L. PETILLA TO SEEK FINANCIAL ASSISTANCE IN THE AMOUNT OF
PHP250,000.00 FOR THE INSTALLATION OF CANOPY FOR THE MINI GYM OF BARANGAY MONTEVERDE,
THIS CITY

WHEREAS, presented for the consideration is to request the Honorable Governor of Leyte
Honorable Leopoldo Dominico L. Petilla a financial assistance for the installation of Canopy of mini-
gym of Barangay Monteverde, Baybay City, Leyte.

WHEREFORE, on motion of Kagawad Mary Angeli G. Debalucos duly seconded by Kagawad

Lorena S. Salibongcogon; be it

RESOLVED, as it hereby resolved to request the Governor of the Province of Leyte, Hon.
Leopoldo Dominico L. Petilla for a financial assistance in the amount of Php250,000.00 for the
installation of Canopy Barangay Monteverde Mini-gym.

RESOLVED FURTHER, to furnish a copy of this resolution to the office of Hon. Leopoldo
Dominico L. Petilla and other offices for information and guidance.
