AFFIDAVIT OF LOSS

I, MARINO M. ALDEA JR., Filipino, of legal age, married and with residence
address at Salvacion St., Cabugao, Bato, Catanduanes, after having sworn on oath in
accordance with law, hereby depose and say:

1. That I owned a motor vehicle which is duly registered and for which the LTO
issued a Certificate of Registration and an Official Receipt.

2. That I needed to present said documents for renewal but I found only the
Certificate of Registration and failed to locate the Official Receipt.

3. That I search diligently for the OR but could not find it anywhere in my files
and I believe that the same is now lost beyond recovery.

4. That I am executing this affidavit to attest to the above facts and for all legal
intents and purpose it may serve.

Baras, Catanduanes, after having sworn on oath in accordance with law, hereby depose
and say:

1. That I owned a motor vehicle which is duly registered and for which the LTO
issued a Certificate of Registration and an Official Receipt.

2. That I needed to present said documents for renewal but I found only the
Official Receipt and failed to locate the Certificate of Registration.

3. That I search diligently for the CR but could not find it anywhere in my files
and I believe that the same is now lost beyond recovery.
