INFORMATION
The undersigned Assistant City Prosecutor, accuses Aldo Monteagudo y Illustre of the
crime of Reckless Imprudence Resulting in Less Serious Physical Injuries and Damage to
Property, committed as follows:

That on or about 24th day of January 2019 at around 8:00 in the evening, in the City of
San Fernando, La Union, Philippines and within the jurisdiction of this Honorable Court, the
above-named accused, was then the driver and person-in-charge of Mitsubishi Montero Elite
SUV, with Plate No. AAA 2409, MV File No. AA147852369, registered under Intrimitida Delavin y
Inocencio, did and there unlawfully, recklessly and imprudently drive, operate and manage the
aforementioned vehicle along Marcos Highway, City of San Fernando, LA Union in reckless and
imprudent manner, without due regard to traffic laws, rules and regulations and without taking the
necessary precautions to avoid injuries to person and damage to property, and by such
recklessness and imprudence, the aforesaid vehicle driven by Aldo Monteagudo y Illustre,
registered to Intrimitida Delavin y Inocencio, causing Honda XRM 125 motorcycle to be damaged
in the amount of Php24,000.00, and as further consequence, Juliet A. Capulet sustained physical
injuries which requires medical attendance for a period of Thirty (30) days barring complications
to her damage and prejudice.
