Forms to be completed

1. UI-19- Employee Information (employer to complete)


2. UI-2.7 Employee Information (employer to complete)

3. UI 2.3 - Maternity Benefits Application (employee and Doctor)


4. UI 2.8 -Bank Accounts Details ( employee and bank)

Supporting documents ( employee).

1. 3 Month’s Bank Statements. Stamped by the bank, not older than three months, (employee).
2. Pay slip for period prior to maternity leave, not after, (employer to provide).

Employer

As an employer, you will need to complete two forms. These are the UI 19 as well as the UI 2.7. (Attached).

UI-19- Employee Information- Completed by employer

The UIF does not accept ANY corrections on the form, even if the mistake is signed, the form will need to be redone.

 As an employer you must complete the Start Date and the Termination Date, irrespective of whether the
Domestic Worker is coming back to work or not.
 The termination date is the last fully paid date.
 The salary on the payslip and the UI 19 must be the same amount.

If the Domestic Worker has previously been on maternity leave whilst in your employ, the employer must reflect this on
their UI-19.

Example

1. First line: Start date of employment, up to the last paid day, before going on the first maternity leave.
2. Second line: First working day after returning from maternity leave, up until the last paid day, before
going on the second maternity leave.

If she is resigning, the code in column H must be a 6 and you will not need to complete a UI 2.7.

UI-2.7 Employee Information completed by employer.

• Please ensure that you show a monthly breakdown of the salary that the Domestic Worker will /will not, be
receiving whilst on leave. In other words the form is future dated.
• This form needs to completed even if you are not paying her a salary during maternity leave.
• This form need not be completed should she have resigned.
• If she is not earning a salary record “NIL” in the last column.
If she is receiving part of her salary, put the amount you are paying in the last column.

An example of a UI-2.7 (Employer).

(There is not enough space for the company UIF number. You can add the extra digits outside of the allocated space)

Date A: 15/12/2011 (First day of unpaid maternity leave , the day after the termination date on the UI 19, regardless of
weekend or public holiday. i.e., if you last paid date (Col G –UI19) is 14/12/11 then this date will be 15/12/11.

Table : First date in the table should be the same date as in A above.

Section B: 15/4/2011 (the date you will probably return to work).

Section C: Do not date.

Employee Section

UI 2.3 - Maternity Benefits Application- Must be original.

• Complete the form in full by providing all personal details and occupation.
• Please do not use Tipex or make any corrections.
• You do not have to complete the ‘occupation code’ or the ‘pay point’ code.
• If you have previous maternity claims, please mention it under ‘previous application’.
• This form must be an original.

SECTION BY YOUR DOCTOR/MID-WIFE:

 Your Doctor needs to confirm your pregnancy by completing the ‘Medical Certificate’ section.

UI 2.8 Banking Details ( Employee)

SECTION BY EMPLOYEE:

• NO mistakes are allowed on this form.


• If your account is in your maiden name, please change it to your married name. Your payments will be blocked if
your account name is not your current /married name.
• Please include 3 months recent bank statement, as proof that the account is yours. The bank statement must
reflect the account holder’s name and the account number. The form must be stamped by the bank.
• The UI-2.8, as well as the bank statement cannot be older than 3 months, on the day of submission.

SECTION BY YOUR BANK:

Please make the bank official is aware that NO MISTAKES are allowed on this form!

No Tipex, crossing out, going over letters, signing mistakes or any other corrections.
