ORIGINAL ABSTRACT
This study use the Job Safety Analysis (JSA), Failure Mode Effect Analysis (FMEA)
and Fault Tree Analysis (FTA) methods. JSA method use to identify the hazards in the
installation process of the tower crane and provide the recommendations on any hazards.
FMEA method used to identify the failures in tower crane’s components and calculate the
value of Risk Priority Number (RPN). Next, the critical component of the tower crane will be
determined by using diagram Paretto. The diagram Paretto will gain a critical component that
will be reanalyzed using the FTA method to determine the cause of the failure. The failure
mode will be used as a top event on the FTA analysis to determine the basic event and
minimal cutset.
The result of JSA analysis shows some potential hazards on tower crane installation
process, which are, occupational hazard altitude, the noise of the pin, the heavy load, corrode
sling mobile crane, the sinking land area of the outrigger, the falling hammer and pin, and not
properly installed sling on the components. While the result of FMEA analysis obtained 14
components with 20 modes of failure. Motor component is a component which has the highest
RPN value with burning motor as failure mode. Based on the FTA analysis, the causes of
tower crane’s component failures are lifetime components, lack of maintenance, pursuit target
work, lack of monitoring and discharging tower crane excessive.
PARAPHRASE
PT.PP (Persero) Tbk. is a construction company that involves a tower cranes which
can cause potential hazards if the malfunction of the component occurs. One of the
component failures is the damage of brake hoist tower crane which could harm the process of
material lifting. Besides, the crane accident occured on the installation, destruction and
section replenishment process. From these incident, hazard identification must be carried out
in the installation of tower cranes and failure of each component of the tower crane and
determine the cause of component failure. This research uses the Job Safety Analysis (JSA)
method to identify hazards in the installation process of tower crane, the Failure Mode Effect
Analysis (FMEA) method to identify failures in tower crane components and the Fault Tree
Analysis (FTA) method to calculate the Risk Priority Number (RPN) value. From these
results of the analysis the recommendations needed to prevent component failure from tower
crane are routine component replacement, maintenance, supervision and avoiding excessive
use of tower crane.
