CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT OF LEASE, and entered into in the Municipality of Cainta,


Province of Rizal, by and between:

, and hereinafter called the LESSOR;

age and a resident of
, and hereinafter called the LESSEE;

WITNESSETH:
1. That the LESSOR is the owner of a residential house situated at

2. That for and in consideration of the agreement herein below stipulated, the
Lessor hereby lets and leases unto the Lessee, and the latter hereby accepts in lease a
residential house under the following terms and conditions:

every day of each month;

c.) That upon the execution of this Contract, the Lessees shall pay one (1)
month advance and two (2) months deposit amounting to

The required deposit shall answer for the payment of any unpaid electric,
water, telephone and other legitimate bills not attended to by the Lessee
or which were not receive by the Lessee at the expiry date of the lease or
for the damages caused to the premises due to the fault or negligence of
the Lessee.

d.) That the light and water consumption, and other facilities used by the
Lessee shall be their exclusive account;

e.) That upon the expiration of one year from date of this contract the Lessee
shall immediately vacate the premises, otherwise, they shall be
considered an unlawful detainee, unless the parties hereto shall renew
the same in writing which must be executed before the termination of this
contract;

f.) That the Lessee shall comply with all the laws, rules, regulation and
ordinances for the preservation of health and sanitation, as well as for the
preservation from fire and other calamities. The Lessee shall
also be responsible for the maintenance and minor repairs that arise on
the daily use of the facilities therein shall be for the sole account and
expense of the LESSEE without the right to reimbursement to preserve
the leased premises from damage, and shall exert all efforts to keep
tenantable character of the leased premises;
g.) That the Lessee shall not introduce any improvement nor make any
alterations without the prior consent of the Lessor;

h.) That the Lessee shall be responsible for all acts or omissions of their
family, servant and any and all other persons allowed by their access to
the leased premises;

i.) That the Lessee shall permit the Lessor or her agents to enter and view
the conditions of the premises at reasonable hours and in a convenient
manner;

j.) That, the Lessee is expressly prohibited to sublet the leased premises to
any one, without the express consent of the Lessor in writing;

k.) That, the Lessee shall notify the Lessor at least thirty (30) days in advance
should the Lessee decide to abandon the leased premises;

l.) That the Lessee shall immediately vacate the leased premises in good
habitable/usable condition upon the expiry of the lease contract without
the need for demand thereof

appeared both parties known to me and to me known to be the same person who
executed the foregoing instrument and ACKNOWLEDGED that the same is her free
and voluntary act and deed.
This instrument consisting of TWO (2) PAGES, including this page where the
ACKNOWLEDGMENT, refers to a CONTRACT OF LEASE all signed by the LESSOR
and the LESSEE and their testimonial witnesses at the foot thereof and at the left hand
margin of the first page.
