Escalona family who is an example of extended or multigenerational family,


resides in Lower Mabini of Barangay Poblacion wherein they live in their own house and
lot. On the other hand, their family type based on task performance is a mix of family
with adolescents and a little of stage of launching their children since some of the family
members also have their own families.

There are 11 members of the family, three of which acts as providers of the
family. An additional member of the family who is currently residing and working in
Kuwait and mother of Raine Zybrine is the eldest daughter of the family named Rosita
Escalona. She also acts as the sole provider of her son; however, she sometimes gives her
share on the family’s expenses if needed. Mr. Roberto Escalona, being head of the family
decides for the welfare of the whole family. In terms of healthcare, Mrs. Escalona is in-
charge most specially with Mary Grace’s condition. The family exhibits close family ties
and there are no observable or reported conflicts between family members. An open and
good communication keeps the family bonded. Mrs. Escalona added that conflicts are
inevitable in every relationship but they readily resolve this conflicts for the benefit of
every members of the family. Problems that are oftenly encountered by the family are
simple misunderstandings and financial matters. Mr. and Mrs.Escalona solve this
problems immediately because they do not want problems to affect their children.

Mr. Roberto Escalona, one of the providers of the family works as a tricycle driver and
Kagawad of their Barangay. Together with his eldest son, Mr. Roberto takes the day shift of
being a tricycle driver and Mark Anthony as the night shift of work, together they earn P400.00
daily. Additional income comes from Mrs. Escalona by cooking and selling breakfast during
weekdays. Mrs. Escalona is the one in-charge in financial plan. The income earned by Mark
Anthony is solely used for the expenses of his 2 children. Mr. and Mrs. Escalona’s combined
income is spent for the whole family.

Rosita sends money for her son monthly. According to Mrs. Escalona, during holidays,
Rosita sometimes also sends money for the whole family.

All in all, according to Mr. and Mrs. Escalona, their income is just enough to sustain the
basic needs of the entire family. If there comes a time that they need additional money, they ask
Rosita if she could help.

Mrs. Teresa added that it is a big help to the family that her children who are enrolled in
college are scholars and supported by organization. Only P2800/sem is being spent as tuition fee
and a little for additional allowance and expenses.

They also utilize the services of Rural Health Centers, District and Provincial Hospitals
for their medical needs mainly because of its free services. Since Mary Grace has epilepsy they
regularly allot money for her maintenance drugs and regular check-up.

All of the family members are natives of Bulacan. The children and the grandsons are all
raised in their own house. Mr. Escalona was not able to finish his studies due to financial
inadequacy when he was still single. Other family members were able to finish High School and
weren’t able to pursue college mainly due to financial instability and loss of interest. Rosalyn
and Clarissa are presently enrolled in College with the assistance of some organizations with
financial and scholarship grants. Mary Grace was not able to enter school due to her medical
condition (epilepsy) and opted to stay home with her mother. The youngest daughter of Mr. and
Mrs. Escalona is presently enrolled in Tibag Elementary school with his nephew Raine Zybrine.
The family is an active member of the Catholic community. Every Sunday the family
goes to church to hear a mass. Mrs. Escalona said that they religiously go to St. Augustine Parish
Church as if it is a requirement for the family.

Significant Others

Whenever the family encounters conflicts and problems, they consult their relatives and
close friends to help them although this rarely happens. Most of the time, they choose to resolve
conflicts and problems within their family.

Relationship of the Family to Larger Community

The family is also active in the community. Mr. Escalona is one of the newly elected
Barangay Kagawad and Kriska Member together with his wife. Having this kind of cooperation
in the community the couple usually attends meetings and programs and as well as participate
programs in the Church. The children in the family are active members of Ilaw organization.

C. Home and Environment

Housing

The family lives in a bungalow type of house made of concrete and wood. The total land
area of the house is 22x14 square meters and the house has 4 rooms, a kitchen, a living room and
combined bathroom and toilet. In front of their kitchen is a public artificial well which is their
source of water for drinking, washing and bathing. They stock water in plastic containers. There
are some low-lying electric cords in the ceiling that may cause harm to the family. A closed
drainage system readily made for the barangay is being utilized by the family where all their
used water is being drained.

Having 4 rooms, the first room is being used by Clarissa, the second room is used by
Mrs. Escalona and Andrea Rose wherein they use electric fan and mosquito coil when sleeping,
the third room is used by Mary Grace, Ronessa, Rosalyn and Raine Zybrine which is located
beside the kitchen area. The fourth room is utilized by Mark Anthony and his 2 sons which is
located beside the bathroom. Mr. Roberto Escalona sleeps in the living room.

The house is not well ventilated in terms lighting and air passages. The entire flooring is
cemented. There is no ceiling in the entire house and the roof in the kitchen and bathroom area is
low.

Their bathroom and toilet is cemented with rough floor. They use a pour type toilet and
stock water using pail and big plastic container.

In terms ofcooking and food storage, they use gas stove and metal kitchen utensils. They
buy their foods everyday since they do not have refrigerator. Their source of water is from the
artesian well where in they stock water through a plastic container. According to them they do
not boil this water before drinking.

In terms of garbage disposal, they collect their trash in a garbage sack and dump in Upper
Mabini where a garbage truck collects it at least twice a week.

Kind of Neighborhood
The family lives in a congested area. The environment is filled with hazardous materials
that may harm and hurt the people. Road and drainage system are poorly established. The
location is also prone to flood and effects of other natural disasters.

Social and Health Facilities Available

Community facilities are available in the barangay. Public market, chapel, Rural Health
Units and Barangay Hall are just walking distance from their house. A nearby Barangay
Elementary School is utilized by the schoolers of the family. Barangay Hall is mostly utilized by
Mr. Roberto Escalona because of his role as a Barangay Official. Other facilities outside the
barangay but still feasible for the family are being utilized such as the St. Augustine Church.

Communication and Transportation Facilities

The family owns a tricycle which is mainly use for their work as tricycle driver. This is
also utilize whenever they need to go elsewhere, buy something and when they bring Mary
Grace to BMC for her regular check-up and the allotted budget from Baliwag to Malolos and
vice versa is P100.00. On the other hand their means of communication is thru cellphone. Their
media communication is through television and radio and sometimes whenever available.

D. Health Status of each Family Member

Medical and Nursing History Indicating Current or Past Significant Illnesses or Beliefs and
Practices Conducive to Health and Illness

During the home visit, all members of the family are in good condition. Not including
Mary Grace, who exhibits a medical condition; members of the family are in their optimal level
of functioning and there are no reports of feeling of sickness.

Cough, fever and colds are the common illnesses encountered by the family. They
usually buy over the counter drugs to cure these symptoms. However, if the toddlers experiences
sickness they sick the help of health care professionals first before giving them medications.
They added that they do not believe in hilot or albularyo.

When Mr. Roberto Escalona experienced UTI 2 years ago, he used TAHEEBO tea for 1
month as an adjunct therapy with his medicines. He said that he feels relieve whenever he uses
this medicine.

Mary Grace visits a neurologist at BMC every month for her regular check-up and take
maintenance medications to prevent episodes of seizures. She seldomly complains toothache and
tried to have her tooth extracted by a dentist but said that it is contraindicated to her condition to
initiate anesthesia.

Dietary History

The family eats 3 main meals a day and snack during the afternoon. They eat breakfast at
around 6:00-6:30 in the morning, lunch at 12:00 in the afternoon and dinner at around 6:30-7:00
in the evening. The family usually eats breakfast that is prepared by Mrs. Escalona and which is
also being sold by Mrs. Escalona. She usually prepares sopas, rice and usual viands such as egg,
hotdog and longganisa. During lunch and dinner, Mr. Escalona prepares the food usually fish and
vegetables of any dish.

Mr. Roberto and Mark Anthony consume 3 cups of rice, Mrs. Ma. Teresa consumes 2
cups of rice during lunch and 1 cup during dinner. Ronessa, Rosalyn,Clarissa and Andrea Rose
consumes 1 cup of rice every meal. Mary Grace, Lyndon Lloyd, John Emmanuel and Raine
Zybrine consumes ½ cup of rice each meal. They only drink water and they do not allow other
beverages because they believe that water is the best for the body. Junk foods are not allowed
inside the house and they only allow the children to have biscuits during snack whenever they
want to during the afternoon.

Eating/ feeding habits/practices

The family washes their hands before and after eating most especially when they use
hand when eating. They use spoon and fork when eating. They do not prayer before or after
meals.

Developmental Assessment of Infant, Toddlers and Preschoolers

John Emmanuel being a toddler can do what a normal toddler can do. In terms of
physical development, he is active, can walk, run, climb, walk up and down stairs alone.
According to his father, he can throw balls and kick them forward, jump with two feet together,
stand on tip toes, take parts of toys apart and put them back together and he starts to show an
interest in toilet training. In terms of intellectual development he can follow simple directions,
use three or more words in combination, express feelings and wishes, have a very limited
attention span, memorizes commercial song advertisements. Lastly in terms of social and
emotional development, he begins to play simple pretend games, self-centered, sometimes do the
opposite of what is asked likes to imitate the behavior of adults and others and he becomes
frustrated easily.

On the other hand Lyndon Lloyd can do what a normal preschooler can do such as to eat
on his own, dresses himself, can draw a man, can do standing broad jump, throws ball overhand,
heel to toe walking, shows early understanding of time concepts through day and night concept,
counts 1 to 10, starts to boast and participate in games with greater number of peers.

Mr. Roberto Escalona is the only family member who have vices. He’s an alcoholic
drinker. He drinks everyday mostly hard liquor and cannot state how many shots/ bottles he can
consume since the drinking session lasts until morning of next day. He also smokes cigarettes
consuming 10-12 sticks of cigarette a day. He started to smoke early 1970s. He tried to stop
smoking in the year 1981-1983. He gradually reduced the number of cigarette stick and
substitute candy to decrease his craving for cigarettes. He resumed his vice in 1984.

During the assessment, all members of the family possess a good physical health. All
family members have scars on their lower extremities due to wound that they had before. Mary
Grace, Andrea Rose, Lyndon Lloyd and John Emmanuel have dirty ears with visible cerumen.
Mary Grace has two open wounds on his right shoulder and elbow. Both wounds can be
compared to a size of one peso coin. She has incomplete set of teeth. She doesn’t have both
upper central incisors, both lower 2nd molars. Lower 3rd molars are decayed. Clarissa also doesn’t
have her right lower 1st molar.

Prevention

Immunization Status of Family Members

Andrea Rose, Lyndon Lloyd and Raine Zybrine are the only FIC among the family
members. John Emmanuel is an X-child.
Healthy Lifestyle Practices

As a part of their healthy lifestyle, the family routinely practices hand hygiene and also
personal hygiene. They keep their diet healthy by having fish and vegetables most of the time.
They seldom drink artificial juices and sodas and limit junk foods in their diet. They keep their
body healthy and tone by doing household chores in everyday life.

Rest and Sleep

The family’s routine for sleeping is usually 10:00 in the evening after watching
television. Mrs. Roberto and Mrs. Ma Teresa wake up at around 3:30 – 4:00 in the morning to
prepare for their work early in the morning. Other members of the family sleeps at around 9:00
in the evening and wakes up at 6:00 or 7:00 in the morning. There are no reports of difficulty of
sleeping which means that they get adequate and continuous sleep. Some members of the family,
Mrs. Escalona and her grandsons usually takes afternoon nap at around 2:00 pm and this last for
one and a half hour.

Exercise and Activities

According to Mrs. Escalona, they do not practice any formal exercises. They consider
their works and household chores as their forms of exercise.

Use of Protective Measures

The only thing that makes them protected at all times is their slippers. They do not use
mosquito nets or pajamas to make them protected while sleeping.

Relaxation and Other Stress Management Activities

The family’s form of relaxation and stress management are watching teledramas and
listeing to radio. Sometimes they also catch on each others stories.

Use of Promotive- preventive Health Services

The family doesn’t have any routine in terms of health activity. They just visit the health
center or RHU if they feel any discomfort or feeling of being sick. However, they utilized the
services of RHU when Lyndon Lloyd, Raine Zybrine and Andrea Rose had their immunizations.
Mrs. Escalona sometimes asks for some medication whenever her Blood Pressure rises.
