The following instructions about completing form F37, and any additional instructions given to you by
the overseas authority/examination centre, must be strictly observed. Edexcel will not accept entries
which fail to comply with the instructions.
A. Before entering for the examination, you are strongly recommended to consult the syllabus
books for GCE Ordinary Level and GCE Advanced Subsidiary and Advanced Level
examinations. These may be purchased from Edexcel Publications, Adamsway, Mansfield,
Notts NG18 4FN, UK. Email address: intpublications@linneydirect.com.
In cases of difficulty, you are advised to speak to your school teacher or British Council.
No adjustments will be made to the timetables to avoid clashes with any local examinations or
occasion.
B. The final date for receipt of entries is 21 October 2011 for the January 2011 examination.
Edexcel will not accept an entry which is received after this date. If entering through an
overseas authority, you must ensure that your entry reaches the overseas authority by
the local closing date so that this requirement may be met. If, for any reason, you decide
not to enter for the examination you must destroy the entry forms, as they will not be accepted
for subsequent examinations.
C. Entry form F37 must be completed and provided to the overseas authority. All the relevant
sections of this form must be completed clearly and accurately with pen (ink or ball point pen).
The entry is not valid unless the form is signed and dated by the candidate.
If you suffer from a disability which might affect your performance in an examination, you
should draw this to the attention of the overseas authority/examination centre at the time you
submit your entry. Medical certificates must be provided where appropriate.
Edexcel will not accept responsibility for any errors or omissions made when completing the
entry forms which are caused by failure to consult the Regulations, Syllabuses or the
appropriate timetable, or by neglecting to read the following notes.
The notes which follow will serve as a guide when you complete the sections of entry form F37
and will help to ensure that you do so accurately.
Notes
1 Having first obtained permission from the overseas authority/examination centre to sit there,
enter the name of the town or city where you are to take the examination followed by the
name of the country in which the town is situated. If the overseas authority/examination centre
requires you to do so, enter the centre name also.
2 Each candidate entering for a modular GCE subject Curriculum 2000 and 2008 GCE AS/A
Level specifications must have a UCI number so that we are able to bank unit scores for future
use.

Candidates who already have a UCI number, or have already taken GCE AS/A2 units or GCE
O Level examinations, must have the same UCI quoted for each exam. You must not change
any part of the UCI which you already hold. If the correct UCI is not supplied, candidates may
not have all of their results credited to them, and may not receive a grade following final
exams.
Candidates entering GCE O and AS/A Level modular subjects for the first time must have a
UCI number generated for them by the centre and should leave this area blank.
It will be essential for candidates moving from one centre to another to know their UCI
number.
3 If you have a Chinese name or a name of Chinese origin and you wish it to appear on the
certificate (if one is gained) in the traditional manner, ie, with the surname appearing first,
the entire name must be written in the space allotted to the surname. If you have adopted
another name and wish that to appear before the surname on the certificate then you
should write it in the space provided for first names. Any name written in the first name box
will appear on the certificate before the surname.
Your date of birth will appear on your certificate should you gain one. It is most important that
dates of birth are quoted accurately. Incomplete dates of birth, eg, 00/08/89, 00/00/89, are
printed on certificates as well as dates of birth quoted in full.
6 For candidates sitting through an overseas authority, two types of examination fees are
payable, namely Edexcel fees and local fees. Edexcel fees consist of a fee which must be
paid for each subject taken. The local fees are determined by the overseas authority and
cover all expenses incurred locally in running the examination.
For candidates sitting through an overseas authority, both Edexcel fees and the local fees
must be paid to the overseas authority in accordance with any directions given at the time of
entry and no entry is valid unless these fees have been paid in full at the time. Should you be
unable to take the examination, Edexcel will give special consideration to the refunding, in full
or part, of the subject fees, provided that the application is submitted on form F20 through the
overseas authority and with its endorsement.
7 You must place a (√) in the box against each subject you wish to take, unless instructed to do
otherwise in the notes below. If there is a choice of papers, you will be instructed to insert an
option code as well as (√), eg:

(a) Ordinary Level Art and Design (7020): Candidates must take one main paper and the
compulsory subsidiary Paper 3. They must choose either Main Paper 1 (Work in Two
Dimensions) or Main Paper 2 (Graphics). The option code, which must be inserted on the
entry form, is dependent on the combination of papers chosen:
Main Paper 1 (Work in Two Dimensions) and Subsidiary Paper 3: Option Code A
Main Paper 2 (Graphics) and Subsidiary Paper 3: Option Code B
Candidates must choose to answer a question from one of three sections in both Main Paper
1 and Subsidiary Paper 3. They may not choose the same section for both papers.

(b) Ordinary Level History: Choose either 7262 Syllabus B (European History) or 7263
Syllabus C (Modern History).
(c) Curriculum 2000 and 2008 Specifications: GCE AS/A2 Modular Units: Candidates intending to
cash in their Unit UMS results for an Advanced Subsidiary or Advanced Level subject grade in
January 2011 must enter with the appropriate cash-in code. The cash-in code must be
entered, in addition to the code for any unit being taken in January 2011. Units taken on
previous occasions should not be entered unless the candidate wishes to enter for the units
again to improve on the results.

(e) Specification cash in codes for New A level Units
(f) The overseas authority/examination centre must be informed immediately should you change
your postal address; neither the authority nor Edexcel will accept responsibility for any
difficulties arising from failure to do so.
(g) Normally you will not be permitted to change your examination centre, but Edexcel will
consider a request to do so provided it is made by, and with the approval of, the overseas
authority responsible for the new centre and that, in the opinion of Edexcel, sufficient time
remains for proper arrangements to be made. A transfer fee will be charged.
(h) Shortly before the examination the overseas authority/examination centre will send you your
statement of entry together with other information for your guidance and instructions which
you must observe. The statement of entry will show your candidate identification number,
which must be used by you throughout the examination, and will give details of the timetable
arrangements. The overseas authority/examination centre will notify you of the time and place
of any practical tests or other examinations not included in the published timetable.
