EMPLOYMENT HISTORY CVS PHARMACY August2008-May 2010 POWELL, TENNESSEE -Worked full-time as overnight Pharmacist; hours consisted working 8pm until 8 a m, seven nights in a row. Responsibilities included, (but not limited to), ente ring, filling, and verifying prescriptions. Counseling patients on all new pres criptions filled. -Multi-tasking was essential and time was of the essence, especially since all Pharmacy Operations and obligations were carried out by myself, due to the fact I worked alone throughout the night. Such obligations involved the greeting of patients; those dropping off new prescriptions, refills, or picking up prescrip tions. Attending to the telephones and to drive through customers/patients to m eet their needs. Giving each patient my attention in a professional and courte ous manner was of utmost importance to me. -Ensuring that the End of Day Reports, (E.O.D. Reports), were complete and print ed out by approximately 2 am. Once this function had been met, I then "brought up" all computer terminals and resolved any issues in the "Que ," such as Third Party Rejects, D.U.R.'s, and faxing physicians for those prescriptions needing r efill requests or Prior Authorizations. -In addition to the above duties; once Reports had run and issues resolved, seve ral pages in the computer consisted of Ready Fills, (automated refills), in whic h I was responsible for printing, filling and verifying. **Reason for leaving CVS was due to scheduling issues; working seven, 12 hour ov ernight shifts.** K-MART PHARMACY May 2008-AUGUST2008 Knoxville, Tennessee -Hired as a part- time Staff Pharmacist with the goal of transitioning to full t ime; provided there would be enough money in the budget -Floated among several stores as a Staff Pharmacist, however,mostly working shif ts at the store in Maryville,TN..Responsible for opening the pharmacy on time an d closing the pharmacy to which I had been assigned to work that day. -Software used was PDX. Responsible for data and prescription entry, while monit oring for any drug interactions, patient allergies and appropriate prescribing. Making certain all customers/patients where counseled with each new prescriptio n while providing pleasant, prompt and professional service. -Additional responsibilities included reviewing wholesale drug orders and makin g any necessary changes, (additions/deletions), to the order before submitting i t; ordering the minimum amount of drugs/merchandise while at the same time focus ing on cost savings. Worked with other staff members, as a team, to ensure pharm acy operations were carried out thoroughly and appropriately. **Reason for leaving K-Mart was due to company not having enough money in their budget to provide me with full-time employment** LAUGHLIN MEMORIAL HOSPITAL October 2007-March 2008 Greenville,Tennessee -Worked full-time as Clinical/Staff Pharmacist. -Clinical duties involved entering orders while monitoring for drug interactions , patient allergies, and/or duplicate orders; software used was Meditech. Addit ional clinical responsibilities included reviewing Lab and Microbiology Exceptio n Reports; monitoring appropriate antibiotic therapy in regards to patient par ameters (i.e. patient weight, height, age, culture & sensitivity and dosage. Rev iewing parameters as ,INR's, Renal function, Platelet count and Aminoglycosid e levels. - Prepared I.V. antibiotics, I.V. admixtures, T.P.N.'s, P.C.A. Pumps. Also, re ady chemo agent, Cubicin, for infusion. Responsible for calculating appropriate Vancomycin dosages for certain patient populations according to State and Hospi tal Regulations Handbook on file at Laughlin Memorial. - Resolving any significant problems issues that would arise; professionally cor responding with Phycicians and other medical staff involving a recommendation of change; according to specific parameters such as renal function and disease s tate the patient possess. **Although I worked at L.M.H. for a brief time, I gained much exposure and val uable clinical knowledge. Reason for leaving was due to lengthy drive each day (1 & hours each way); Pharmacy Director was aware and understood reason for lea ving.** Rite Aid Pharmacy February 2007 - SEPTEMBER 2007 Oliver Springs, Tennessee - Hired as full time Pharmacy Manager at store in Oliver Springs,TN -Performed financial duties ,such as ensuring Standard Pharmacy Operations were carried out thoroughly and appropriately; keeping wholesale costs down, monitor ing and ordering inventory; making any necessary changes to "computer generated" warehouse order. - Additional responsibilities included helping to increase prescription sales, a chieving specific goals set by the company, focusing on customer service and sat isfaction; In June 2007, the pharmacy I managed won "Best Customer Satisfaction" in the district. -Staff scheduling, delegating Pharmacy Technician duties, establishing work flow and providing excellent patient care were among other responsibilities of mine. ** Reason for my leaving Rite Aid was two fold; I no longer desired working as t he Pharmacy Manager for that particular company and I made the decision to chang e my scope of practice as to gain more exposure as a Clinical Pharmacist** Kroger Company April 2006 - November 2006 - Hired as full-time Floater Pharmacist, performing tasks such as filling and ve rifying prescriptions, counseling patients and providing excellent customer serv ice. -Worked as a Team Player in carrying out assigned tasks accurately, effectively and promptly. - Ensuring "work flow" was carried out appropriately and efficiently among staff . **Left Kroger due to there not being a permanent placement at one particular sto re with the company** Walgreens Company May 1999 - March 2006 -Worked as a Staff Pharmacist for several years at various Walgreens stores. -Promoted to Pharmacy Manager in 2001. Requirements included, but not limited to , carrying out managerial duties such as scheduling, delegating tasks to staff a nd encouraging cross training among pharmacy staff to help ensure a more "well r ounded" pharmacy team. Providing excellent customer/patient care. - Monitoring inventory through various methods such as routine "cycle counts", o rdering and/or returning drugs/merchandise to maintain an appropriate on hand ba lance. -Achieving K.P.I. goals, (Key Performance Index), set by the company. Reviewin g the pharmacy's performance, (based on K.P.I. Reports), each week, so as to "fo cus" on particular areas that may need improving;(i.e. increasing sales of gener ic drugs; decreasing amount of time entering, filling and verifying prescription s to enhance work efficiency and work flow leading to better customer service; d ecreasing wholesale items purchased that could otherwise be provided by company warehouse. -Returned to the position of Staff Pharmacist, (at my request), for approximatel y two years and worked as a Floater Pharmacist for the remainder of my employme nt with the Company. **Left Walgreens due to wishing to seek employment with company whom had hours t hat were more conducive to my family/personal life. macy; Atlanta, Ga. December 1999 CLINICAL FOCUS- (NOT LIMITED TO) -COUMADIN ROTATION * Completed 6 week Clinical Rotation at Blount Memorial out patient Coumadin Cli nic. Involved dosing Coumadin to new and established patients; monitoring requi red parameters such as I.N.R.'s, P.T. time, side effects, and others. Dosage a djustments were not only based upon such parameters, but also from making such a complex decision. * I extensively counseled approximately twelve patients a day, one on one, empha sizing important patient information; compliance, side effects, foods and medica tions to avoid, and much more. Encouraging patients to ask questions and my fee ling sure each patient understood my counseling was essential . This was carrie d out professionally and with great compassion.
