Agreement / Declaration
1. Powertech IST Otokon, a division of Powertech SA (Pty) has made this Subscription Agreement available for use by anyone who wishes to subscribe to the Silk AMR Hosting service. 2. Each person who requests the Silk AMR Hosting service is required to complete this application form. 3. Your application is subject to the Terms and Conditions of Powertech IST Otokon which is accessible on the Silk website. Those Terms and Conditions may be published and amended by Powertech IST Otokon from time to time. 4. Powertech IST Otokon reserves the right of final decision in any dispute. If your request is accepted by Powertech IST Otokon, you will be given access to Silk and you will be bound by a Subscription Agreement as set out in your Subscription Agreement Form on specified Terms and Conditions. 5. Subscription to Silk must be for at least 24 consecutive months. 6. Payments are to be made one month in advance. 7. Please provide all the required information on the Application Form. 8. Please detach the original form and submit it to Powertech IST Otokon, alternatively to an Otokon agent or an Otokon accredited Service Provider, by hand or by fax to 012 426 7320 (Attention: AMR Hosting). 9. We strongly recommend that you use a Powertech IST Otokon accredited service provider to install the energy meter s and modems. 10. Please allow 10 working days for your account to be activated.
