Reliance Accidental Death & Total and Permanent Disablement Rider for enhanced cover The death benefit payable will be equal to the Rider Sum Assured opted for, if death occurs directly due to an accident. The Rider also pays disablement benefit (i.e if the life assured is totally and permanently disabled due to an accident), which is equal to base sum assured paid in ten equal annual installments. In addition to the above disablement benefit, the company will waive all future premium under the base policy and rider up to maximum of Rs.40,000 p.a. How does the Plan Work? ILLITERATE 100,000 The customer pays premium every year. The Premium paying Term is always 5 years less than the Policy Term. + refers to incremental vested bonuses, if any that will be paid along with the death benefit or accidental death benefit * Bonuses are paid only when declared WHAT IS ACCIDENTAL DEATH & ACCIDENTAL TOTAL AND PERMANENT DISABILITY Accidental death is loss of life directly as a result of an accident and intimated within 90 days of occurrence Total and Permanent disablement is defines as total and irrecoverable loss of sight of both eyes or loss by severance of two limbs at or above ankle or wrist or total and irrecoverable loss of sight of one eye and loss by severance of one limb at or above wrist or ankle for a period of at least six months. This should have happened directly as the result of an accident.

DISCONTINUANCE OF PREMIUMS If discontinuance happens after payment of at least three full years premiums: Policy will be converted into a PAID-UP policy for a reduced Sum Assured. Life insurance protection continues to the extent of the paid-up value until the end of the policy term but rider benefit will cease. Any Accumulated Bonuses attached to this Policy will remain attached in full. However once the Policy becomes PAID-UP no further bonuses are payable. The Benefits payable would be as follows: On Survival to the end of the Premium paying term, Paid up Sum Assured will be given On survival to the end of the Policy terms, the vested bonuses shall be paid In the event of loss of live the Paid up Sum Assured plus vested bonuses shall be paid If less than three years premiums have been paid: Policy LAPSES at the end of the grace period & Insurance Cover and Rider benefits cease immediately.

No benefits will be paid when policy is in lapsed status.

POLICY REVIVAL Policyholder may revive a policy anytime before the date of maturity Revival is subject to meeting the terms and conditions of the company.

Extra premium @ Rs1.50 per thousand sum assured for Non Standard Age Proof needs to collected in addition to the basic premium. Declaration for paying the age extra premium in view of Non Standard Age Proof must be attached. IMPORTANT CHECKLIST: 1. Dont leave any column blank in the proposal form. 2. 3. 4. 5. 6.
