A Certificate of Insurance to show General Liability is in force in the amount of $1,000,000.00 including coverage for premises and product liability. 2. Sales Tax Number or Social Security Number:______________________________________ 3. Electricity Provided: 110 Plug, 30 amps, and 220 plug, 50 amps (Vendors should have their own internal breakers). 5. There are no water drains in the park, vendors must bring their own receptacle for water disposal. 6. Place all garbage/empty boxes in the large refuse containers provided at the edge of the park and not in the rollways or containers placed in the food court. 7. Concession Booths may set up and open on September 9th after 1:30pm and must remain open until 6:00pm on Saturday, September 10th. 8. Vendors wishing to stay open later than 6:00pm on Saturday, September 11th, may do so at no additional charge. (There is entertainment that evening.) 9. Concession Vendor will be assigned a 15' x 20' space. The 20' section will be the front of the booth. Concessions larger than 15 x 20' must pay for two spaces. 10. Food vendors will be located on the south end of Rotary Park, Highway 152 and Mustang Road. 11. No beer sales allowed. No selling along streets or alleyways. 12. Please do not contact the Chamber office for the location of reserved space(s), on September 9th looks for the white tent with a banner that says Mustang Chamber.

AGREEMENT As an exhibitor, I agree to protect, defend, indemnify and hold harmless the Mustang Chamber of Commerce, its members or directors from and against any and all claims, damages, losses, liabilities, causes of action, suits and expenses of litigation of every kind and character without limit and without regard to the cause(s) thereof or the negligence of any party or employees, or exhibitors sub-contractors or their employees, on account of bodily injury, death or damage to property, unless otherwise provided herein. Exhibitor understands this is an outdoor event, inclement weather could force cancellation. No rain date is provided, and there will be no refunds for any reason. Space is limited; mail entries in early to ensure proper electricity requirements. Exhibitor is responsible for all sales tax due on their sales. Entry deadline August 20, 2011.
