Computer Software & Hardware Business Maintenance Agreement

This is a prepaid agreement between recipient ___________________________________________________ & the provider, Alpha & Omega Computer Repair, to provide services as outlined below to ____ computers on-site at the recipients location______________________________________________. This maintenance agreement establishes a process for the routine hardware and software maintenance of customers computers and programs, which include hardware maintenance, operating system updates, and antivirus software updates. Cost of replacement/upgrade parts, if needed, is not included.

We will contact you once every 60-90 days (on a pre-arranged schedule) to set a maintenance appointment. No need for you to remember when to do it. While there, we will clean your computer(s), removing any spyware, adware, malware, viruses, and general computer "clutter" that accumulates over time and slows down your system's performance. Since dust accumulates inside your system (preventing critical components from cooling properly), we will physically clean your system as well. We will update all operating system and program files, and optimize your system for best possible performance. We will back up your files and data per a pre-arranged backup protocol and verify that your backup system(s) are functioning properly.

Fees: We will charge $59.95/month for one system, $20/month for each additional system Service
Contract Termination by either party is permitted, as long as 30 days notice is given in writing.

Procedure for non-maintenance issues:

When a user feels that they have a hardware or software problem with any of the covered computer equipment, an email, or phone call should be made to the Provider for repair service. Upon receipt of the service request, we will respond by the next business day, and sometimes on the same day. Non-maintenance repair work will be done at 25% off of the normal hourly rate

Others
We will not be responsible for any of your accounting, book keeping, or data loss. Cost of any third party software licensing i.e. Antivirus, Windows, MS Office, etc. is not included in this agreement. If number of computer listed on the contract is exceeded a $20.00 per additional computer per month will be added, and in case it decreases from the range mentioned above, the contract amount will be adjusted accordingly.

The terms of this agreement starts on _____/_____/2011 and will last for 1 year at the fixed rate of $__________/month.
We hereby agree with the terms & conditions contained in this Hardware/Software Maintenance agreement.
