than .8" and is therefore not a significant hazard. No report is requested at this time.

Complaint forward to the FLS and IIC as notification upon request of OPHS.
Contacted the complainant. She states that while she was eating a hot dog (2nd bite in) she noticed a piece of blue plastic embedded
in the link. SHe [eft the piece of plastic in the hotdog. Aside from feeling quesy there was no other iIlness or injury associated with the
complaint. She has agreed to send in a picture of the hot dog and the foreign object. She also has two other packages which she
purchased at the same time. She is planning on looking through them for other evidence of plastic in the hotdogs.

The caller purchased the product about 1 week before. The caller's daughter found a dark, grey, green spot in the center of the hot

80 beef frank hotdogs. The complainant stated he found the beer bottle cap inside the bottom of the box,
where the 80 beef frank hotdogs were separated inside a plastic bag. Apparently, the metal beer bottle cap

with the EIAO.
The EIAO asked if she still had the product and she said no. She had taken pictures of the product with the
camera on her cell phone. The EIAO asked her if she had taken any pictures of the can showing the
establishment number or of any identifying marks on the can. She stated that she had not. She said that she
took a picture of the one "maggot" that she thought was still alive and of the numerous others still in the
product. The EIAO asked her to forward the pictures she took on her phone to the EIAO's blackberry. The
pictures would not transfer. The EIAO asked her if she saw any visible damage to the can, and she stated that
she had not. The can was not dented or swollen. She said that she had prepared half of the can for her son
and then when she went back for the rest of it, noticed the maggots in the remaining half. She called the
hospital and spoke to the poison control center to determine if they would have a reaction to eating maggots, if
in fact they had consumed any. The personnel there said that there was no health risk.

Notes :It appears as if someone has ejaculated in these hot dogs and would like to report
it. I sure hope I'm crazy because I ate a couple of them. I opened the package the day I boutht them,
so they should have been fresh.
I like the hot dogs, they're my favorite brand and I was so disappointed.
I have some bowel symptems, possibly due to my colon problems, not sure if related to the hot
dogs .. just wanted to alert someone about this. thanks.

EIAO had the following phone conversation with complainant on 1/15/2009 between 0843 and 0857.
Complainant stated that she purchased Ball Park Frank (1 pound package with 8 hot dogs to the
package) in the middle of September 2008 from Wa[mart in Big Rapids, Ml. The establishment
number is 756A. The sell bye date is September 25, 2008. Complainant put the package in the
freezer. In early October 2008, Complainant took the package out of the freezer to chip off 2 hot
dogs for her and her husband. When she flipped the package over, she observed a silver steel
looking item that had a half moon shape about a quarter of an inch long. She did not open the
package, and she contacted Sara Lee Foods. Sara Lee Foods informed her that they would send a
mailer to complainant to retrieve the package. complainant still has the package and has not had
any further contact with Sara Lee Foods. Complainant said that because of Holiday Season, she
would wait and hear from Sara Lee, but she did have any further contact. She was told to contact'
USDA by coworkers.

Original Statement: Consumer bit into a frankfurter (fully cooked and smoked flavored) resulting in cracking a tooth and
a tooth filling fell out on 07/22/2008. On 07/23/2008, she observed a black jagged sharp piece of
black metal or hard plastic protruding from one side of one of the frankfurters. This object was 112
inches. She called the point of purchase and notified them of the incidient. She called the 800
number on the- package of the product and was told by the person that the FDA allows a certain
percentage of contaminants to be in those products.
Case Closing Reason: Investigation resulted in an NR for the day the product was produced. The lie report is
complete and no further action will be requested from OPHS for now. We wi!! continue to
closely monitor for complaints from this establishment. Case will be archived and
reevaluated should a similar complaint be reported or additional information comes forward.

Review of PSIS data within the last 6 months revealed the following NRs in the identified plant:
NR 5120077235 Foreign Material Incidence Report of 06/14/2007 documenting the finding of metal fragment
from the chain of the neck splitter in a tank of turkey necks at the POSS grinder and the establishment failed to
implement any corrective action in response to finding metal contamination.
NR 50-2007-7235 regarding an appeal for POSS metal detection record indicated that metal was detected that
was greater than 7 mm. in dimension and no record of corrective action in response to the deviation from
critical limits as required by regulation.
NR 29-2008-8631 regarding observation of metal shavings on meat pieces in the.
NR 2920087174 regarding the establishment failing to document the 3 part SSOP corrective action on
3-29-2008 when metal shavings were found on thigh meat pieces.
Further information with the complainant will assist in determining if this NR is relevant.

[8/4/20083:20:00 PM1 Visited complainant on 8/1/2008 to verify complaint and product information, and to observe and photograph
product and foreign object. Complainant would not permit Investigator to remove object from individual turkey frank. Investigator was
unable to determine the material of the object (metal, plastic etc).
Visited POP on 8/1/2008 and spoke with Manager, indicated that upon being notified by the complainant
during the week of July 20th, she removed 4 packs of turkey franks from the store retail display and placed them in storage.

I ( EIAO), contacted the FLS and IIG of Establishment 00582, and asked them look into
this issue. The IIC at this establishment reported that there is nothing documented in the plant's records that
would indicate past incidences or issues that would lead to finding a foreign object in product. He also stated
that he had not observed anything that would lead to this issue. The IIC reported that the plant only uses green
rubber gloves for production of this product.
With no code number, date, net weight, or lot numbers it makes this incident extremely difficult to investigate.
During his investigation the IIC discussed the complaint with the plant, and they wanted to know if the rubber
glove was found in the wiener or in the batter of the corn dog. Also the IIC stated that the plant does not make
a "Brown Beef Corn Dog" and wanted to know if it should have been "Bryan Beef Corn Dog".

Today a customer painted out a package of Oscar Mayer XXL beef hotdogs with a metal object in it
on the shelf at our commissary. The Oscar Meyer beef hotdogs were immediately pulled off the shelf
(32pkg) with the same UPC. The Kraft Sales rep gave us a case number 17506844 and said they
would call us next week. In the meantime, the hotdogs will remain on Med hold pending further
disposition or review. There was only one package with the visible piece of metal. Public Health is
working the issue at the local level.

Notes :An FSIS employee (works for Center for Learning, public health officer) bit into a
hot dog; looked down in to the hot dog and saw a soft piece of plastic like the type they use for
bread, thin plastic. Looked circular maybe about the 14 -1/2 inch in diameter with a line of black
ink/printing on the plastic. No injury noted.
The hot dog was served at the FSIS Unity Day event 6/12/08 at the GWCC/USDA building by a
catering company.
The caterer in charge is who said the product came in institution style shrink
wraplvacuum packaging. The product information is on the box, which she did not have. So at this
time we don't have any identifying information for the product. The caterer is going to try and track
the box down, but at this time the name of the catering company is unknown. The contracting person
for the event is from Acquistions & Agreements with FSIS. She may have
the name of the contracting company ( I am in the process of trying to get the name of the catering
company from her)

I spoke to the complainant. She stated that while boiling the hotdogs, she found a piece of wood floating in the water. Then while eating

Spoke with complainant. He indicated that his sone was eating a hot dog when he started choking on a white piece of plastic made of
the same material as the wrappers. No injury nor illness resulted. He also was eating a hot dog when he began choking as well on a
foreign object of the same type. It was about 1-2 inches long. Complainant also indicated that the labeling had no expiration date. He
spoke with a representative from the company and they tested the first plastic piece and it was determined that it was indeed a wrapper
and not casing. Complainant has product on-hand.

COMPLAINT PURCHASED A CORNDOG FROM WEINERSCHNITZEL. THE COMPLAINT'S SON

The establishment is 61378 P. The last complaint 5309-2007 resulted in an lie review. Given that thisis the third such complaint OPHS
requests that this complaint be forwarded to the IIC for an investigation into the processing practices at the plant that may lead to
plastic being found within the corn dogs.

Title: Case 2007-5643 EIAO ESTABLISHMENT
Establishment submitted a report in response to CCMS cases 5643-2007 and 5609-2007. According to the report, the in-house
investigation revealed that the problem was derived from the stainless steel (S/S) thrust collars on the frank stuffing machine. The
establishment has replaces the SIS thrust collar with a plastiC thrust collar.
Report also indicated that, no product from the "pack date" in question was available on inventory for sampling. However, similar

products and evaluate the presence of casing fragment in product). Establishment reported that, result of the sampling did not reveal
presences of casing/casing fragment in product sampled.
Establishment proposed to monitor the stuffing equipment for the next two weeks including, daily collection of 23 -30 franks per day for
evaluation.
Contatced the complainant. She states that this incident occurred in MArch 2007. Her son bit into a com dog that contained a large
piece of plastic within the dog. There was no injury associated with the complaint although he was taken to the doctor for treatment of
food pOisoning. According to the Health Hazard Evaluation Board on bones and foreign objects, particles greater than 0.8 inches in
size may pose a safety risk. Given the shape of the object thereis a concern for a possible choking hazard.

Title: Case 2007-5643 COMPLAINANT
Complaintant was referred to USDA Office from LA. Couty Health Department. L.A. Couty Health Department conducted an inspection
of the Weinerschnitzel Restaurant on March 20, 2007. They day after the complaint was received at their office. Once referred to the
USDA office in Diamond Bar on March 21, 2006, EIAO attempted contacting the conplaintant 4 times and left messages
with no success. returned messages on April 4, 2007. AUhis point, E;IAO, was given the complaint
to invesitgate.
On March 30, 2007, , L.A. County Health Inspector, faxed her report to EIAO regarding this complaint.
Enclosed in the report were findings of issues with holding temperatures for hot product.
On March 21, 2007, took her son, , who has consumer part of the corn dog to visit their Family Practioner,
at had been
experiencing diarrhea and cramping for two days. diagnosed with gastroenteritis and prescribed
Amoxicillin, an antibiotic, and Lonox, an antiperistaltic and anticholinergic. During my phone conversation with she
indicated that her son did not go to work that week due to his illness. She indicated that her son did not have any blood tests or fecal
analysis conducted by the doctor.
EIAO spoke with the owner of the Weinerschnitzel restaurant to determine the origin of the corndog receieved by the
Weinerschnitzel. Health Inspector, also provided a invoice with information regarding the origin of the hotdogs, obtained
during her initial inspection. After speaking with General Manager of Prepared Foods at Foster Farms in Livingston,
CA. He confirmed that Foster.Farms, Est. 06137B-P in Livingston, CA, in the only Foster Farms establishment that produces Galardi
Brand corndogs for the Weinerschnitzel Corporation.

Complaint could not be verified because evidence has been discarded. OPHS asks that this case be shared with the IIC due to the
recurrence of this complaint and the potential for health hazard. No report is requested at this time.
This morning, EIAO received the instruction of investigating the case from FLS Dr. who provided
detailed information about product and complainant. EIAO immediately contacted the complainant and let her know
that if EIAO could meet her and verify the defected product Turkey Franks including taking photos for the foreign object.

said to her that she could throwaway. She said that the product was purchase about two month ago at "Food 4 Less" on South Damen
Ave. and it was not boughtfrom "Fair Play." She went on and stated that she was advised by her lawyer to make the complaint. To
follow up the case, EIAO went to POP "Food 4 Less" store located on South Damen and interviewed the Meat Department Manager
acknowledged that he has never received any complaint about the product. EIAO randomly picked up
a pack of product from the display shelf and purchased a pack of Turkey Franks (Foster Farms, Est. P-6137B, Net wr 16 oz, sale by
April 23, 2007). Subsequently, EIAO opened the package breaking the hot dogs did not found any foreign subject.

data within the last 6 months revealed no noncompliances that suggest a direct relationship between this complaint and practices in the

I spoke to the complainant. She described finding soft, clear plastic marked with a black line down the side inside of the hotdog. No

Complaint could not be verified because evidence has been discarded. OPHS asks that this case be shared with the I IC due to the
recurrence of this complaint and the potential for health hazard. No report is requested at this time.
This morning, EIAO received the instruction of investigating the case from FLS Dr. who provided
detailed information about product and complainant. EIAO immediately contacted the complainant Ms. and let her know
that if EIAO could meet her and verify the defected product Turkey Franks including taking photos for the foreign object.

said to her that she could throwaway. She said that the product was purchase about two month ago at "Food 4 Less" on South Damen
Ave. and it was not boughtfrom "Fair Play." She went on and stated that she was advised by her lawyer to make the complaint. To
follow up the case, EIAO went to POP "Food 4 Less" store located on South Damen and intelViewed the Meat Department Manager
acknowledged that he has never received any complaint about the product. EIAO randomly picked up
a pack of product from the display shelf and purchased a pack of Turkey Franks (Foster Farms, Est. P-6137B, Net 'NT 16 oz, sale by
April 23, 2007). Subsequently, EIAO opened the package breaking the hot dogs did not found any foreign subject.

Searched GeMS by establishment number and found no similar complaints reported within the last year.
Review of PBIS data within the last 6 months revealed no noncompliances that suggest a direct relationship between this complaint
GeMS history indicates that rodent droppings and worms are commonly reported for this type of product, and usually shown to be burnt
batter. Based on this history and the absence of rodent-associated NRs in PBIS, this case will be closed. Letter explaining disposition

Attempted to contact complainant to clarify complaint description, without success. Left voicemaiL
I spoke to the complainant. She described finding black material inside of the corn dog breading which resembled rodent droppings.
She reported not observing an unusual odor. She noted that she experienced diarrhea after eating the corn dog and attributed it to the
product. I explained that these types of complaints are not unusual and previous complaints have been investigated and found to be
the result of bumt batter.

This complaint reports that a foreign object may have caused the reported illness. Search of CCMS complaints over the last year show
several other foreign object complaints with similar "characteristics" including the possible "rodent dropping". In the past it has been
determined that burnt batter can give the appearance of rodent droppings in this type of product. There was one illness complaint 1
year ago which was not confirmed and never investigated.
Complaint reviewed. Will contact the complainant next week.
Title: Case 2007-5328
Contacted the complainant. She tates that the product was consumed on December 14, 2006 and the child (18 months old) became ill
on 12/15/2006. At the time, the complainant had recently given birth to her own child. She and the nephew were ill with watery diarrhea.
Hers resolved quickly while his lasted about 2 weeks. No testing was done he was only treated with pedialyte the first day. In addittion,
this was the first time that this product was introduced to the child.
Contacted the POP and they denied any other complaints.

[4/29/2008 5:27:41 PM] Spoke to complainant 4/24/08. She stated that there were 6 sausages in the package
and she fed hed daughter one of them before noticing what appeared to be a dragonfly inside the packaging.
The appearance and smell of the actual product was normal. The daughter also became ill with nausea,
vomiting, and diarrhea for 3 days. There was a doctor's visit, but no stoll samples were taken. Amoxicillin was
prescribed.
