WARRANT OF ARREST

TO ANY OFFICER OF THE LAW: GREETINGS: You are hereby commanded to arrest (name of the accused), accused in the above captioned case what is said to be found at (complete address) and who is charged with (see caption) and to bring said accused before this Court as soon as possible to be dealt with in accordance with law. The bail bond for his/her provisional liberty is hereby fixed at (amount) which may be furnished by the said accused either by deposit the amount thereof, with the nearest collector of Internal Revenue or Provincial City or Municipal Treasurer who shall transmit the same to this Court, or by furnishing a personal bail bond, subscribed by two or more sureties whose solvency may be shown by their tax receipts and certification of title free from all liens and encumbrances to be determined by any Judge of the Regional Trial Court or Municipal Trial Court within the 10th Judicial Region, who may accept the bond, qualify the bondsmen and the transmit the same to this Court. The Peace Officer making this arrest is hereby authorized to discharge the accused from custody after furnishing the bail in the manner herein above indicated which must state in the return hereof. The bail bond shall embody the stipulation, signed by the bondsmen and the accused to the effect that should the accused jump bail and/or fails to appear during trial, the same will be deemed an express waiver of his/her/their right to be present during trial, as if the accused were present without prejudice to an Order for his/her/their arrest and/or confiscation of the bailbond.
