Photography Release Form

For good and valuable consideration, the receipt of which is acknowledged, I, hereby grant the NKBA Ottawa
Chapter or its authorized representative, the non-exclusive right to the use the photography referenced below to
promote the NKBA Ottawa Chapter, its services, competitions, and its members through any and all media
including, but not limited to, various print pieces as well as broadcast and internet opportunities without
restriction as to alteration. I further waive any rights to inspect or approve the finished product or products and
the advertising copy or other matter that may be used in connection with the NKBA Ottawa Chapter, Vision
Design Awards or the use to which it may be applied.
The NKBA Ottawa Chapter will provide credit to all photography published and reserve the right to crop or alter
the photography at its discretion or as necessary in creating any one of the above stated projects.
I warrant that the photography is original and my own creation and does not violate any third party agreements. I
release and agree to hold harmless and indemnify the NKBA Ottawa Chapter, it licensees and assigns from all
claims, demands, causes of action, damages, liabilities of any kind and reasonable attorneys fees which may arise

Design Statement(s) must be typed and should not include any identifying
member or client name, business, or location information. Narrative should consist
of 200-400 words that describe your project entry in the best way. List highlights,
objectives, design challenges/solutions, and special features. For more tips of

We highly encourage all members of the NKBA Ottawa Chapter to enter their design work
into the Canadian design competition (see NKBA Ontario Chapter) AND the national
competition hosted by the NKBA (link to information above).
