Each Avago supplier is asked to return a completed copy of this Social Responsibility Self Audit Checklist to appropriate Avago contact prior to the Avago on site audit.

Purpose:
This self audit check sheet is designed to offer Avago suppliers a method of ensuring compliance with the Supplier Social Responsibility Code

Instructions:
1. Fill in general information sheet 2. Begin completing the checklist by reading the statements in column B. 3. For each statement in column B, enter 1 for "True" or 0 for "False" in column C. Should a statement not apply to your business or operations, enter "1" in column D. 4. Input applicable reference documents associated with your answer in column E. 5. Applicable comments and references shall be added in column F. 6. Return the completed Self Audit Check sheet to your Avago contact prior to the Avago on site audit.

We ensure our contractors, suppliers and consultants are aware of the Avago Supplier Social Responsibility requirements. 2. Compliance with Laws and Regulations We comply with all international, national, regional and other applicable laws and regulations that apply to the region in which we operate. We provide adequate information and training to all employees regarding relevant legal, regulatory and internal requirements that apply to their jobs.

3. Basic Human Rights 1. Equal Employment Opportunities We work according to a documented and established policy on no discrimination in the 1 recruitment, training, promotion, dismissal or general administration of our employees. We work according to a documented and established policy prohibiting sexual, racial or 2 religious slurs, jokes, comments and behavior. 2. Fair Employment Arrangements All employees are employed based on a 1 personalized, written agreement between the employer and employee. Employment terms, including wages, working hours and benefits in our company 2 are fair and compliant with the legislated minimum level requirements. Employees are granted their relevant statutory rights such as annual leave, sick 3 leave and maternity/paternity leave without any form of repercussions. We work according to a documented and 4 established policy of no forced, bonded, involuntary prison or illegal labor. Employees are free to leave their 5 employment after reasonable notice. We work according to a documented and 6 established policy of no physical abuse or discipline. We work according to a documented and established policy of no threat of abuse, 7 including sexual or verbal harassment or intimidation.

3. Child Labor We comply with the legislated minimum age for employment of the country in which we operate. If there is no legislated minimum level, we enforce a minimum age of 15* for 1 our employees. *14 years of age is acceptable under the circumstances specified in Article 2.4 of ILO Convention No. 138 on Minimum Age for Admission to Employment. 4. Safety Requirements 1. Safe and Healthy Working Conditions We provide a safe, healthy, pollution free and productive environment to ensure the well1 being of our employees and the quality of Avago products. Adequate steps are taken to prevent 2 workplace accidents and injuries to the employees. 2. Facility Safety 1 The production area is clean and safe. Emergency preparedness procedures 2 including evacuation plans are communicated to all employees. There exists adequate fire extinguishing 3 capabilities. Evacuation plans are clearly posted 4 throughout the work facilities. 5 Emergency exits are available in all areas. Emergency exits are clearly displayed and 6 kept unobstructed at all times. 5. Fair and Honorable Business Practices 1. Respect for the Environment The Avago Supplier Environmental Social Responsibility Code of Conduct, found at http://www.avagotech.com/quality/ are 1 recognized within our organization and there are programs in place to meet those requirements. 2. Promoting Fair Competition and Procurement in the market place Suppliers and contractors are selected on the basis of competitive price, quality, delivery, 1 etc. but not on the basis of inducements, bribes or preferential treatment.
