PROBLEM Inability to recognize the presence of health threat due to lack of knowledge about their present condition Subjective cues from the clients: Ganito ba talaga ang nararamdaman ng mga nagbubuntis? Hindi ko alam kung ano ang mga dapat kong gawin ngayong buntis na ako. Tuwing kalian ba kailangan magpakonsulta sa doktor? Anu-ano ba ang kailangan kong kainin para maging ligtas at malusog ang baby ko? Nakakatulong ba ang pag-eehersisyo sa pagbunbuntis ko? FAMILY NURSING DIAGNOSIS Inability to decide about taking appropriate actions due to failure to comprehend the identified problem as a health threat during pregnancy GOAL OF CARE After 2 weeks of community nursing interventions, pregnant clients of Central Signal Village Health Center shall be able to comprehend problems during pregnancy as a health threat OBJECTIVES OF CARE After 1 week of community nursing interventions, pregnant clients of Central Signal Village Health Center will: a. recognize the risk factors that will contribute to the identified b. identify the different measures to prevent the arousal of the risk factors of the problem c. gain knowledge about the importance of having a healthy lifestyle during pregnancy d. formulate own decision making strategy about taking appropriate actions to identify problems encountered during pregnancy INTERVENTIONS > Discuss desired activities and any modification necessary for pregnancy with client > Consult with clients primary and maternal health care providers about safety of overthe counter (OTC) medications > Explain the need for ultrasound exam to assess for fetal growth restriction > Suggest measures to combat nausea and vomiting, such as dry crackers > Help client locate at least one person she can depend on if a pregnancy emergency should arise RATIONALE >Finding a match between desired activities and preferred activities will help with adherence > Not all OTC medications are safe during pregnancy EVALUATION After 1 week of community nursing interventions, clients are now aware of their present condition and are more knowledgeable on the risks it imposes towards pregnancy; are able to verbalize concerns experience that may hinder in having a healthy pregnancy and be able to identify barriers that contribute to the problem as well as measures to be taken with such problems.

> Teratogen exposure may have slowed fetal growth, which will be revealed by ultrasound exam > Decreasing nausea to increase food intake aids fetal growth and development > Pregnancy can be a very stressful time for a woman who has an insecure support system

Data from clients are as follows: Some clients just had their first prenatal check-up on their second trimester of pregnancy. Some clients eat whatever they want to eat regardless of their present condition. Some clients are concerned about their health. Some clients do not know the importance of prenatal/postnatal exercises
