HOSPITAL PROFILE Dr. Luther Z. Ramiro and Mrs. Dinah L. Ramiro founded Ramiro Community Hospital. RCH opened its door to the General Community on February 13, 1975. It has an authorized bed capacity of 100 beds with actual operating capacity of 120 beds excluding 2 bed general intensive care unit, 8 Bed NICU, 3- bed Labor Room. It is an accredited Tertiary Level of care and second Level Referral Center.

JOB SUMMARY Receives the previous shift. Listens and takes note of the endorsement. Does the morning round to the patients. Assists the doctors during rounds and in any surgical procedure. Relays to the doctor any results in the laboratory exams. Asses each patients health status, response to treatment, ability to provide self-care, coping with chronic illness and function in relation to the activities of daily living. Identifies and responds to the nursing needs of the patients by proper observation Coordinates and refers to the charge nurse or senior nurse all matters that concern the patient. Collaborates with the patient, family, physician and other health care professionals Acts as an advocate for the patient and the family. Makes sure that the room and the set-up are ready before the patient is transported to the room. Provides and maintains adequate facilities, equipments, medicines and supplies for good patient care. Maintains safety of the patient. Promotes comfort and safety in lifting, positioning, restraining and transporting the patient by maintaining proper body alignment and utilization of proper body mechanism. Takes and records patients vital signs such as blood pressure, pulse rate, respiratory rate, temperature and weight. Measures accurate intake and output of the patient. Keeps complete and accurate information reports and records.

Performs nursing activities such as catheterization, suctioning and oxygen administration per doctors order. Secures consent and makes sure that the patient has clearly understood about what had the doctor explained regarding any procedure to be done such as any surgical intervention. Attaches monitoring devices such as the cardiac monitor, pulse oximeter and blood pressure apparatus. Trains and supervises volunteer nurses under his/her care. Does proper recording of cases and charting which a must following the standard of safe is nursing practice. Completes the chart of the patient by filling up items related to the treatment procedure. Does proper endorsement to the next shift.

DETAILED DUTIES AND RESPONSIBILITIES as a MEDICATION NURSE Remains competent in the knowledge of medication administration which includes but is not limited to the following areas: 1. Appropriate response to medication errors, lack of drug effects, and negative outcomes Administers the right dosage of the right drug at the right route to the right patient at the right time Prepares and gives intravenous medication aseptically Stores medication in the original containers as dispensed by the pharmacy and makes sure that all pharmacy labels shall be intact and legible and shall not be altered Properly stores all regulated drugs to a safe place Educates the patient and the family about what the medications are for and as to what possible side effects may occur Notes any reaction to the medication and records them properly Notifies the charge nurse, supervisor and to the doctor any negative effects of the drug Applies medication to the patient whether by ingestion, inhalation, insertion, sublingual, topical or parenteral as ordered by the doctor Does proper assessment paying particular attention to the laboratory results, vital signs and neurological status of the patient before administering drugs that can cause an increase or decrease in the patients vital signs and metabolism such as cardiac drugs and insulin injections Assess IV site for any signs of phlebitis Changes and regulates Intravenous Solution per doctors order
