Dear Nurses, We are fourth year BS Pharmacy students at Pines City Colleges conducting a research entitled Impressions of Nurses

Towards Working with Pharmacists in Baguio City Hospitals. We want to determine the awareness, expectations, barriers and acceptance of nurses as they work together with pharmacists. In this regard, we are requesting for your help in answering all the items in this questionnaire. Rest assured that your answers will be treated with high confidentiality. Providing advice on special disease state modified dosing (e.g. renal, hepatic). 3. Providing consultation regarding alternative medication choices and medication interactions. 4. Providing consultation regarding medication use in pregnancy. 5. Providing consultation regarding adverse effects and resolving drug-therapy related problems. 6. Advising on availability of medication and making medication decisions based on medication pricing. 7. Assessing patients contraindications to therapy. 8. Monitoring outcomes of drug therapy and treatment plans. 9. Conducting a mandatory review for highly risk medication/ patients. 10.
