Orbital contents, skin of nose or cheek , minimal extension to ant.cranial fossa , ptyregoid plates , sphenoid or frontal sinuses T4b : tumor invades any of the following : orbital apex , dura , brain , middle cranial fossa , cranial nerves other than V2 , nasopharynx or clivus -------------------------------------------------------------------------------------------------------Maxillary sinus T1 : tumor limit to the maxillary sinus mucosa with no erosion of bone T2 : tumor causing bone erosion or destruction including extension into the hard palate and / or middle meatus , except extension to posterior wall of maxillary sinus and pterygoid plate T3 : tumor invade any of the following bone of posterior wall of maxillary sinus , subcutaneous tissue , floor or medial wall of orbit , pterygoid fossa , ethmoid sinus T4a : tumor invade anterior wall of orbit contents , skin of cheek , pterygoid plate , infratemperal fossa , sphenoid or frontal sinus T4b : tumor invade any of following : orbital apex , dura , brain , middle cranial fossa , cranial nerve other than V2 , nasopharynx or clivus

Oral cavity T1 : tumor < 2 cm T2 : tumor > 2 cm and < 4 cm T3 : tumor > 4 cm T4 ( lip ) : invade cortical bone , inferior alveolar nerve , floor of mouth or skin of face T4a ( oral ) : invade adjacent structure ( cortical bone ,deep tongue musculature , maxillary sinus or skin of face ) T4b ( oral ) : invade masticator space , pterygoid plate , skull base or encase the ICA -----------------------------------------------------------------------------------------------------------Oropharynx T1 : tumor < 2 cm T2 : tumor > 2 cm and < 4 cm T3 : tumor > 4 cm T4a : tumor invade larynx , deep extricsic muscle of tongue , medial pterygoid , hard palate or mandible T4b : tumor invade lateral pterygoid muscle , pterygoid plate ,lateral NP , skull base or encase carotid artery N1 : single , ipsilateral < 3 cm N2a : single , ipsilateral > 3 cm and < 6 cm N2b : multiple , ipsilateral < 6 cm N2c : bilateral or contralateral < 6 cm N3 : > 6 cm -----------------------------------------------------------------------------------------------------------Thyroid gland T1 : < 2 cm T2 : > 2 and < 4 cm T3 : > 4 cm or any tumor with extrathyroid extension ( sternothyroid , perithyroid soft tissure ) T4a : any side beyond thyroid capsule to invade subcutaneous soft tissues , larynx , trachea , esophagus or recurrent laryngeal nerve T4b : tumor invade prevertebral fascia , encase carotid artery or mediastinal vessel All Anaplastic Ca are T4 T4a : intrathyroid resectable T4b : extrathyroid unresectable N1 : metas to level 6 ( pretracheal , paratracheal , prelaryngeal , Delphain node ) N2 : unilateral , bilateral , contralateral cervical or supraclavicular or mediastinum LN ------------------------------------------------------------------------------------------------------------Salivary gland T1 : < 2 cm without extracapsular extension T2 : 2-4 cm T3 : > 4 cm or extraparenchymal extension T4a : involve skin , mandible , ear canal , or facial nerve T4b : involve skull base , pterygoid plate , encase carotid artery

Hypopharynx T1 : tumor limit to one sub site of the HP and is 2 cm or less in greater diameter T2 : tumor involve more than one sub site or and adjacent site or > 2 and < 4 cm without fixation of hemilarynx T3 : tumor more than 4 cm or involve fixation of the hemilarynx T4a : tumor invade thyroid cricoid cartilage , hyoid bone , thyroid gland , esophagus or central compartment soft tissue ( prelaryngeal , strap muscle , subcutaneous fat ) T4b : tumor invade prevertebral fascia ,encase carotid a. Or involve mediastinum structure ------------------------------------------------------------------------------------------------------------Supraglottic T1 : tumor limit in one sub site and normal cord mobility T2 : tumor invade mucosa more than one sub site of supraglottic or glottic or reginal sub site of supraglottic ( BOT , vallecular , medial wall pyriform ) without fixed of laryngx T3 : tumor limit to larynx with vocal cord fixed or / and invasion of any postcircoid , preepiglottic , paraglottic space , and /or minor thyroid cartilage erosion T4a : tumor invade through thyroid cartilage and / or invade tissue beyond the larynx ( tracheal , soft tissue of neck , including the deep extrinsic muscle of tongue , the strap muscle , the thyroid or esophagus ) T4b : tumor invade the prevertebral space encase carotid a. or involve mediastinal structure -------------------------------------------------------------------------------------------------------------Glottic T1a : tumor limit to one vocal cord T1b : tumor involve both vocal cord T2 : tumor extend to supraglottic and / or subglottic with or without impaired vocal cord T3 : tumor limit to the larynx with vocal cord fixation and /or invasion of paraglottic space and / or minor thyroid cartilage erosion T4a : tumor invade through the thyroid cartilage and / or invade tissue beyond the larynx ( trachea , soft tissue of neck , include the deep extrinsic muscle of tongue , the strap muscle ,thyroid or esophagus ) T4b: tumor invade the prevertebral space , encase carotid a. or invade mediastinal structure ---------------------------------------------------------------------------------------------------------Subglottic T1 : tumor limit to the subglottic T2 : tumor extend to vocal cord , with normal or impaired mobility T3 : tumor limit to the larynx with vocal cord fixed T4a : tumor invade the cricoid or thyroid cartilage and / or tissue beyond larynx ( trachea , soft tissue of neck , including the deep extrinsic muscle of tongue , strap muscle thyroid or esophagus T4b: tumor invade the prevertebral space , encase carotid a. or invade mediastinal structure
