Waiver: In consideration of being permitted to participate in any way in the Sports Program (Tournament), I, for myself, my heirs, personal representatives or assigns, do hereby release, waive, discharge, and covenant not to sue Hmong Family Community Alliance Church, from any and all claims resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in the Sports Program (Tournament). Assumption of Risks: Parlicipation in the Sports Program (Tournament) carries with it ceftain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. and activities will be reported to the authorities. No dogs allowed on school campus. No smoking on campus. Please smoke at the parking lot or street. No fire-arms [gunsJ and or weapons allowed on school campus. Proper Authorities will be notify No fighting on school campus. Any fights will disqualify the team[s) involved from the event with no money back. Help keep the school clean and use the trashcans.
