MODEL RELEASE FORM

I hereby agree to supply you Photographs/digital images for inclusion in Bizarre magazines Ultra Vixens. I hereby warrant, confirm and agree that:
1. In consideration of your agreeing to consider publishing my Photographs/digital images I hereby licence you, Dennis Publishing Limited ( D PL) of 30 Cleveland Street, London W1T 4JD the non-exclusive worldwide right to publish in perpetuity the Photographs/digital images in Bizarre magazine and any DPL publication and/or its licensees including as part of any advertising, marketing and promotion venture in all media now known or hereinafter invented including but not restricted to distribution on platforms such as mobile and online. For the avoidance of doubt, I understand your usage will include using the Photographs/digital images to promote Bizarre magazine and any DPL publication by using the Photographs/digital images in the national/regional press and television campaign. I understand that all Photographs/digital images supplied to DPL will not be returned to me. I warrant, represent and confirm that I have the full power to enter into this agreement and to give the warranties and indemnities contained in this agreement and that in the event that the consent of any third party consent is required that I have obtained all such consent. I hereby agree to indemnify DPL and keep DPL indemnified fully against any loss, injury or damage (including any legal costs or expenses and any compensation costs or disbursements) which DPL in its absolute discretion agrees to pay to settle or compromise any claim occasioned to DPL in consequence of any breach or alleged breach of the warranties given herein or otherwise arising from DPL s usage of the Photographs/digital images. DPL can use all or any of the Photographs/digital images as DPL in its absolute discretion and I irrevocably and unconditionally waive all rights to which I am now or may be in the future entitled pursuant to the provisions of the Copyright Designs and Patents Act 1988 or to the extent permissible pursuant to any other similar or equivalent legislation now or in the future enacted under the law enforced in any part of the world. I understand that nothing contained in this release or otherwise shall be deemed to require you to publish the Photographs/digital images. I confirm that I am over 18 years of age. I warrant that DPL shall be free to sublicense all rights in the Materials and the indemnity contained herein shall extend to such assignee(s) and sub-licensee(s). This Agreement shall be governed by the laws of England. I have read and fully understood the terms of this contract
