1. Issuance:

a) ESI & P.F. no. of contract worker
b) Gate pass number of worker
c) Work order extension

3. Cancellation
a) F&F Settlement (EL, Bonus & Wages) Gratuity*
b) Service Certificate Copy
c) Enhanced Wages Paid
WORK COMMENCEMENT
1. EI: Intimation to P&A the within 3 days (Annexure A)

2. Contractor to open file in P&A:


a) Labour License
b) Form VI-A: Within 15 days
c) Register of Workmen
d) Employment Card: Within 3 days
e) ESI and EPF Code
f) Muster Roll

3. GM-PP: Form VI B-Within 15 days
