CASE PRESENTATION

Background Information
 Zelda Jacobs bon on July 20, 1964 in Lumberton, North Carolina.

 She grew up on the farm with eleven siblings. Worked one mill job but mostly worked in restaurants.

When working in restaurants, she worked in management. She would hand the billing and payments for

receiving food deliveries, pay checks for employees, handed complaints from consumers, and place food

orders.

 She married at 16 and had four kids with her husband. Now she has 8 grandchildren. Enjoys spending

time with her family and is the head of her intermediate family.
Problem Statement
 Here for short-term care, she broke both of her ankles. Was sleeping on the couch then got up to use the

restroom but fell and broke her left ankle first. Since she was in shock, kept walking and fell again to

break her right ankle.

 Had to have surgery on both ankles but the right ankle need to have steal pates since it was twisted the

wrong way. The surgeries were done one at a time.

 Nervous breakdowns, bipolar, and depression. With her depression, she could days without showering or

eating or doing anything productive.


Services
 Is receiving physical therapy, upper and lower (arms and legs).

 Skilled nursing services, salon services, pedicure and manicure services, and some counseling.

 Independent smoker at Genesis Pembroke Center.


Strengths and Limitations
 God, good cook, outgoing, friendly, determined, and took care of herself.

 Antisocial, depression, and no motivation.

 Her daughter sometimes has to help her and remind to be productive. Like laying out clothes for her to

get dressed for the day. She lives alone and does not drive.
Assessment Goals
 To motivate her to start walking while using a walker or cane then eventually on her own.

 Making progress everyday with the help of the physical therapy doctor to push her.

 Gain muscle, become independent and increase her self-esteem.

 Try new coping skills.


Specific Interventions and Time Frame
 Attend Physical Therapy for 30 days.

 Attend daily social activities and interact with other residents and staff.

 Be able to walk with a walker or can instead of using a wheelchair for two weeks.

 Start walking on her own and to walk up and down a ramp.

 Biases: She could days without showering, hygiene and changing her clothes for day.

 Differences: I grew up with one brother, love to drive, and independent.

 Life experiences: Being separated from her husband and having nervous break downs.

 Staff interventions: Attending physical therapy on a daily basis to build muscles.

 Research: Activities keep residents thriving especially if the activities are meaningful and different types. Helps

with the quality of life for residents.

 Changes to Agency: Have more different type of activities for residents to participate in.
