Background Information
• Client is a 29-year-old, single, African-American male
• Father of 3 young children (1 in foster care)
• No known criminal history
• Tested positive for marijuana July 2018
• Last drug screen
Problem Statement
• Child is currently in foster care. Plan is reunification with child.
• Barriers:
• Substance abuse
• Currently has a 1 bedroom apartment
• Financial status
• Has only attended 1 supervised visit out of 9
Services
• Freedom House

• Help manage mental health disorders
• Help recover from addictions and maintain sobriety
• Offer individual, group, and family therapy

• Client attend Freedom House scheduled for every Wednesday from 1-2pm until
service is no longer needed
• By next court date, client will demonstrate progress on current living situation
• Client will attend supervised visits with child beginning next week on 10/23/18

• What were some value differences between you and the client? Does not believe in therapy/seeking
services
• What events or life experiences have shaped them into the person they are? Parents have passed away;
in and out of foster care; never sought mental health treatment
• What is the staff recommendation about diagnosis, interventions or treatment? Freedom House; drug
screenings is NOT treatment
• What does research say on their diagnosis/problem/treatment? Has been very successful in the past
based on statistics from Freedom House
• What changes do you recommend for the agency or service? Follow-up more with clients (GAL)
