Gordons Functional Pattern :

PATTERN Before Hospitalization During Hospitalization


Patient FR was confined to the hospital (date) due to severe pain in her right leg that started (date) when she slipped and feel on her (part of the body) While doing her laundry without her crutches. She had her right hip fractured and undergone surgery (partial hip replacement) under general anesthesia last (date) and is expected to recover from her present condition with the help of the health care providers attending to her needs.

ANALYSIS

Health Perception

Patient FR is a 69 year old female diagnosed with a type 2 diabetes and hypertension. The patient went to a Leftleg Transtibial amputation surgery under general anesthesia last 2003 due to necrosis in her left foot. Patient FR complies with her medical needs such as daily bp monitoring , blood glucose monitoring. All the medications that are prescriptions to patient FR is being followed she was taking Imdur,glucophage,isosorbide and other medications. Annually visits the doctor for regular check-up

Partial hip replacement refers to the surgical replacement of the hip joint by a prosthetic implant.

Trans-tibial amputation refers to the complete removal of the lower segment of the leg below the knee. Amputation is considered to be drastic and traumatic experience to other people

Before patient FR was admitted she weighs around (lbs) and her usual diet is a sugar-free,low-carbohydrate meal this is usually a cup of rice with soup and vegetables/fish and sometimes meat for lunch or dinner. Patient often takes snacks as she desires in between meals but only in moderate amount. During her confinement in the hospital patient FR was instructed to be NPO due to Pre-Operative SOP and was inserted with IV with (type of fluid).

Nutritional and metabolic

An individuals health status greatly affects eating habbits and nutritional status. patient FR was admitted to the hospital due to severe pain in her right leg because of the fracture from her right femur.

Patient FR usually voids 6-7 times a day with no difficulty and defecates every 2436hrs. Patient FR had a catheter inserted w/ml of urine insider the urine bag and was given a fleet enema before surgery. after the surgery patient FR had her urine bag filled with (ML) of urine and is being monitored for G.I activity

Even though patient Fr had lost her left limb she can still walk and function normally with the help from her crutches or her artificial limb to do ADLS and other household choirs. Other activities such as for leisure

Patient FR usually sleeps around 9-10 pm after watching tv and wakes up around 4-5 in the morning. Patient FR takes short naps in the afternoon when she feels tired. Patient FR complains about difficulty in sleeping during the time that she got her right leg injured she says that the pain from her left leg Refreins her from sleeping and she does not feel comfortable even when shes lying down.

Sleep rest

Illness that causes pain or physical distress can result in sleep problems. people who are ill require more sleep than normal and normal sleep and wakefulness is often disturbed. There is a sleep disruption of sleep and wake cycle because the patient is in pain.

Patient FR can speak both english and tagalog that can be well understood. she can read and write properly. Because of patient FRs present condition and after having general anesthesia upon assessing on her level of conciousness she reponds to voice and responds to pain.

Patient FR can speak and understand english and tagalog. she has 2 daughters one is married and the younger one has just graduated from nursing. Patient FR is very active when it comes on taking care of her family and doing household choirs. When patient FR was confined and had undergone surgery her family was all there to take care of her and support her.

Patient FR is a member of the iglesia ni cristo. According to patient FR she goes to church every Thursday and sundays for mass and prays before sleeping. Patient FR has complied with all medical regime and there are no practices that affects her hospitalization she has a strong faith in god and prays for her fast progress.
